CLOSED Helluva Halloween Giveaway Master Form

As I mentioned in my Helluva Halloween Intro post, I am doing things a little differently this year by having a master form for all of the giveaways.  This means that you enter your information in the form, and then when a giveaway comes along and you want to enter, you just leave a comment on that post with whatever the post asks for - no need to leave additional info.
Filling out this form does not enter you automatically in any of the giveaways.  Also, you only have to fill out this form once!
This is only so I don't have to repeatedly gather your information.  I'm cutting out a step and saving us all some headache (hopefully).
